ChartLock provides AI-assisted documentation tools intended solely to support licensed clinicians in drafting clinical documentation. ChartLock does not create, author, validate, or submit medical records and does not provide medical, clinical, legal, or billing advice.

By accessing or using ChartLock, you acknowledge and agree that:

1. User Is the Author of Record

You are the sole author and responsible party for any documentation you enter, copy, paste, modify, or submit into any electronic medical record (EMR/EHR) or clinical system.

2. Mandatory Review and Verification

You are solely responsible for reviewing, editing, validating, and approving all generated content for accuracy, completeness, clinical appropriateness, regulatory compliance, and billing correctness prior to submission.

3. No Clinical Decision-Making

ChartLock does not make clinical decisions, recommendations, diagnoses, treatment plans, or medical judgments. All clinical decisions remain entirely your responsibility.

4. Assistive Technology Only

All content generated by ChartLock is assistive in nature and may contain errors, omissions, or inaccuracies. AI-generated output is not authoritative and must not be relied upon without independent professional review.

5. Assumption of Risk

You assume full responsibility and liability for the use of any documentation generated through ChartLock, including any clinical, legal, regulatory, billing, malpractice, or compliance outcomes.

6. Limitation of Liability

To the maximum extent permitted by law, ChartLock disclaims all liability arising from or related to the use, misuse, or reliance upon generated documentation, including but not limited to documentation errors, omissions, or downstream consequences.

7. Professional Responsibility

Use of ChartLock does not alter your professional obligations, standard of care, or legal responsibilities as a licensed clinician.

8. Account Security and Single-User Access

Your account is licensed to a single user and must not be shared; password sharing or allowing others to use your credentials is prohibited and may result in suspension or termination.
